Unit 3 Glossary
International Classification of Diseases, Tenth Revision, Clinical Modification — U.S. standard for diagnosis coding. First section of ICD-10-CM code book, organized alphabetically by condition name; starting point for code lookup. Second section of ICD-10-CM containing codes in alphanumeric order with notes and instructional directives. Instructional note indicating mutually exclusive conditions that cannot be coded simultaneously. Instructional note indicating the excluded condition is not included here but may be coded additionally if present. Used in ICD-10-CM codes to fill empty character positions so a required 7th character falls in the correct position. Not Elsewhere Classified — indicates no specific code exists for the documented condition. Not Otherwise Specified — equivalent to 'unspecified'; used when documentation is insufficient for a specific code.
